What it does
An MCP server that lets AI agents create, edit, validate, publish, and manage forms, quizzes, surveys, and workflows in Formasty. It exposes 75 operations for building and operating production-ready forms with conditional logic, webhooks, and response handling.
- Generate lead-capture forms, intake forms, or application forms from natural language descriptions.
- Build conditional workflows and quizzes with routing rules, variables, and scoring logic.
- Publish forms and retrieve submission data, exports, and integration assets like embed codes and webhook URLs.
Before you enable it
An MCP server runs with whatever access you give it. Start with a development account rather than production, and check which tools the server exposes before you let an agent call them unattended.
